FORM FOR APPLICATIONS RELATED TO THE LAW ON THE PROTECTION OF PERSONAL DATA (LPPD)

OVERVIEW

Natural persons (referred to hereinafter as the “Applicant”), whose personal data is processed are described as “data subjects” in Law No. 6698 on the Protection of Personal Data and are entitled to making certain demands regarding the processing of their personal data, as stipulated in Article 11 of the same Law.

Pursuant to Article 13 (1) of Law No. 6698, applications to be made to the WIM ASSOCIATION, the data controller, regarding exercising of these rights must be submitted in writing or by the other methods specified by the Personal Data Protection Board (the “Board”).

Accordingly, for applications that are to be made in writing to our Association, applicants shall send their requests to the addresses given below by one of the following methods:

Personally by the Applicant;

Notification through a public notary; or

E-mail signed by the Applicant with a secure electronic signature defined in Law No. 5070 on Electronic Signatures.

When any other methods are determined by the Board, the means of sending applications using those methods will be announced accordingly.

Pursuant to Article 13 of Law No. 6698, the application submitted to us will be responded to according to the content of your application through the method you prefer after the necessary procedures are finalized within thirty days following the receipt of your application by the WIM ASSOCIATION.

This application form is designed to determine your relationship with the WIM ASSOCIATION, to ensure the complete identification of your processed personal data, if any, and to allow the association to reply accurately and on time to your application regarding your rights listed in Article 11 of Law No. 6698. To eliminate the potential for legal risks that may arise from the illegal and unfair sharing of data, and particularly to ensure the security of your personal data, we, the WIM Association, reserve the right to request additional documents and information deemed necessary to determine your identity, your connection with the WIM ASSOCIATION and/or your personal data, and to verify your authorization for application. The WIM ASSOCIATION accepts no responsibility in the event of the submission of wrong or out-of-date information regarding your requests on the form, or for unauthorized applications.
